How to Apply
Application Process
There is a two-step application process to apply for the Anchor Collaborative Cohort. Planning grants for developing a full application are available; individuals or teams can access these grants by requesting one when they submit their Expression of Interest (EOI).
Step 1: Submitting an Expression of Interest
Before completing a full application, teams should submit an Expression of Interest by one of the 3 intake dates (no later than March 26, 2021).
How to Submit an Expression of Interest
Submit your EOI via the McConnell Foundation’s online portal, called Fluxx.
Start by accessing the portal and creating an account. Please note: it can take 1-2 business days for accounts to be activated before you can begin to complete your EOI.
Collaborate with your team using this worksheet to develop your responses. Then, the lead applicant should paste final responses directly into the online Fluxx form. All applications must be submitted through Fluxx in order for the Selection Committee to review.

Step 2: Submitting a Full Application to the Nourish Cohort
Once you’ve submitted your EOI, you can proceed with developing a full application. Teams must submit their full applications by April 27, 2021. If you have not previously submitted an EOI please contact the Nourish team before submitting an application.
There are 3 things required for the full application:
Completed responses to all application questions (questions here; final responses must be submitted via Fluxx)
Completed spreadsheet (downloadable here) showing status of stakeholder engagement (Tab 1) and readiness to participate in the cohort (Tab 2)
A letter of support from a senior leader (e.g. CEO, President, Senior VP) in your lead health care organization

Submit your application via the same McConnell Foundation online portal used to submit your EOI, Fluxx
Access your portal account using the same login credentials created at the EOI stage

Eligible Applicants
Each EOI should have a minimum of two applicants: a health care lead organization and a community partner organization.
Health care organizations eligible to submit include: hospitals, health authorities, long-term care, Community Health Centres, Indigenous Health Access Centres and other Indigenous health services. Community partner organizations can include: community food organizations, Indigenous organizations or governments, grassroots community groups, local climate or environmental groups, service organizations, or other community organizations that are partnered with the health care organization.
Full applications should build upon the EOI by expanding the list of diverse partners and team members who have come on board during the recruitment period.
